About Corewell Health South 

The health system comprises three hospitals, a regional cancer center, rehabilitation centers, two long-term care residences, home care and hospice services, and 34 affiliate physician practice locations. We provide both inpatient and outpatient medical services throughout Southwest Michigan.  

  

Scope of Work

Provides quality education, counsel, and support to clients and their significant others regarding diabetes self-care. Documents the education process, patient information and clinical recommendations according to department/program/regulatory guidelines, and measures outcomes to maximize continuity of care between settings. Participates in projects related to area of specialty and the development/revision of procedures/policies, educational materials/guidelines, educational programs. Collaborates with and serves as a resource for other health professionals to promote delivery of quality services and patient education within the institution as well as the community.

Qualifications
  • Required Bachelor's Degree Nursing or related healthcare field

  • Preferred Master's Degree

  • 3 years of relevant experience recent, relevant clinical experience including prior teaching experience with adult clientele Required

  • LIC-Registered Nurse (RN) - STATE_MI State of Michigan required Or

  • CRT-Registered Dietitian (RD) - CDR Commission on Dietetic Registration required

  • CRT-CPR - AHA American Heart Association 90 Days required

  • CRT-Diabetes Educator (CDE) - NCBDE National Certification Board for Diabetes Educators 3 Yrs required
