Job Profile Summary
Role:
Under RN supervision, the Per Diem - LPN is responsible for providing nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit.

Qualifications:

  • A high school diploma or G.E.D. or an equivalent combination of education and experience
  • Current LPN licensure in the state of employment
  • Active BLS for healthcare professionals from the American Heart Association or Red Cross.
  • For current Hope Employees: BLS should be obtained by January 1, 2025
  • For current Capital Caring employees: BLS should be obtained by November 1, 2024
  • Mobile Driver - Valid driver’s license and automobile insurance per Company policy
  • Reliable transportation to meet visit schedule
  • If working in an inpatient unit certificate of completion of 30 hour Infusion Therapy Course and in-house 8-hour Infusion Class preferred
  • Ability to communicate (written and verbal) in English
  • Flexible schedule to meet the needs of patients
  • Ability to use equipment with visual and auditory mechanisms
  • Ability to manage the emotional stress of working with and caring for terminally ill patients and their families
  • Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation
  • Can work within their scope or practice per state regulations

Job Responsibilities:

  • Under the direction of an RN, provides nursing tasks to patients and families such as continuous care, dressing changes, catheter changes as identified in the plan of care developed by the Interdisciplinary Group (IDG).
  • Demonstrates clinical skills regarding application of Company policies and procedures such as:  medication administration, symptom management, and patient/family relationships.
  • Observes/monitors patients for alterations in body systems, pain and symptoms and reports to RN.
  • Observes and reports signs of adverse reactions and responses to treatments and reports to RN.
  • Reports findings to physicians, APRNs and physician’s assistants following collaboration with RN.
  • Performs venipuncture for blood specimens.
  • Performs Central Venous Line (CVL) site observation/inspection;  venous access device dressing, cap change and flushing venous access device; IV medication and fluid administration, IV pushes (per procedure) and venipuncture.  May not make any changes to infusion pump including, but not limited to, rate changes and IV bag changes.
  • Documents per policy and procedure in medical record.
  • During times of emergencies (i.e. Hurricanes, etc.), the LPN may be required to report to work at a location designated by the company, to ensure continuity of services. This may include reporting to work ahead of your scheduled date/time due to planned lock down of unit, and staying overnight(s) based on duration of emergency
  • Performs other duties as assigned.

Some locations may require:

  • Cross training to Hospice House positions of equal licensure/certification and scope when census calls for flexible staffing.
