$20.31 - $27.42The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities:
Works closely with the Provider team to facilitate admission or transfer of patients from Strong Memorial Hospital Emergency Room to UR Medicine affiliate hospitals. Interviews and consents patients for transfer, answers patient and family questions regarding transfer to affiliate locations and completes appropriate forms for transfer. Interfaces with care team members including physicians, nurses, social work, and support staff to facilitate communication. Creates transfer center intake for service requests to be triaged and managed by the appropriate area leaders.JOB DUTIES AND RESPONSIBILITIES:
