🏥 How You’ll Make an Impact as a Speech-Language Pathologist

As a crucial member of our home‑based care team, you will:

• Champion Elara Caring’s mission and philosophy
• Assess and evaluate patient needs, establishing a personalized Plan of Care
• Perform initial OASIS assessments and ongoing re-evaluations
• Ensure patient safety through thoughtful care planning and resource coordination
• Collaborate with physicians, supervisors, and interdisciplinary team members
• Implement and revise Plans of Care based on patient progress
• Provide direct speech-language pathology services according to clinical standards
• Select therapeutic materials tailored to patient needs
• Educate and support patients and caregivers on home therapy techniques
• Maintain accurate, timely documentation and communication
• Work in a dynamic, physically demanding environment with flexibility for irregular hours

✅ What You’ll Need

• Master’s degree from an accredited program in Speech-Language Pathology
• Current, unrestricted SLP license in Indiana
• Minimum 1 year of clinical SLP experience
• Ability to travel 50% within your service area
• Reliable vehicle, valid driver’s license, and auto insurance
• Ability to lift 50–100 lbs. and assist with patient transfers
