Home Health Physical Therapist (PT)

Full-Time | Field-Based
Coverage Area: Lapeer North to Kingston

What You’ll Do as a Physical Therapist:

• Deliver high-quality, patient-centered physical therapy in the home setting

• Evaluate patient needs, develop individualized plans of care, and adjust treatment to support optimal outcomes

• Collaborate with interdisciplinary team members to ensure coordinated care

• Educate patients and caregivers to support continued progress at home

• Provide leadership and guidance to support staff when needed

• Maintain timely, accurate documentation in accordance with standards of care

• Serve as a positive ambassador for Elara Caring within the community

What We’re Looking For:

• Graduate of an APTA-accredited Physical Therapy program

• Active, unrestricted PT license in the state of practice

• At least 1 year of experience in a healthcare setting

• Comfortable working independently in a field-based role

• Willing and able to travel up to 50% within the coverage area

• Reliable vehicle, valid driver’s license, and current auto insurance

• Ability to perform physical demands of the role, including lifting up to 50–100 lbs
