Deliver high-quality, patient-centered physical therapy in the home setting
Evaluate patient needs, develop individualized plans of care, and adjust treatment to support optimal outcomes
Collaborate with interdisciplinary team members to ensure coordinated care
Educate patients and caregivers to support continued progress at home
Provide leadership and guidance to support staff when needed
Maintain timely, accurate documentation in accordance with standards of care
Serve as a positive ambassador for Elara Caring within the community
Graduate of an APTA-accredited Physical Therapy program
Active, unrestricted PT license in the state of practice
At least 1 year of experience in a healthcare setting
Comfortable working independently in a field-based role
Willing and able to travel up to 50% within the coverage area
Reliable vehicle, valid driver’s license, and current auto insurance
Ability to perform physical demands of the role, including lifting up to 50–100 lbs
