POSITION OVERVIEW:  

The Consult Liaison (CL) Psychiatrist is responsible for providing consultation to the emergency room and inpatient floors for patients with primary psychiatric conditions or psychiatric comorbidities. The CL psychiatrists works in a team environment that includes master’s and PhD level clinicians performing risk evaluations and helping to stabilize patients in acute psychiatric crisis.  This position will interface with Inpatient Behavioral Health services, which includes twenty inpatient beds, to coordinate patient care and admissions to the units. This position will provide M-F coverage of Psychiatric Assessment Team (PAT), providing psychiatric consults primarily for northern campuses of Redmond, Madras and Prineville with a combination of telehealth and in person consultation based on patient complexity and presentation. Cross coverage of Bend consults may also occur when Northern campus volumes allow.  This position will collaborate with psychiatric leadership to develop and maintain an Electro Convulsive Shock Therapy (ECT) service line that will be new to St Charles.

DEPARTMENTAL SUMMARY: The division of Behavioral Health (BH) services at St. Charles Health System serves four communities with both inpatient and outpatient services. Each of Bend, Redmond, Madras and Prineville have a hospital system in conjunction with outpatient clinics. Within these systems, the division of BH services is divided into integrated Behavioral Health Consultants into primary care, outpatient specialty services, inpatient services, and PAT consultations that occur in the emergency departments and hospital inpatient medical settings.  St Charles Health System includes the only, inpatient, acute care psychiatric beds in Central and Eastern Oregon.  We have 5 Psychiatric Emergency Service (PES) beds and 15 inpatient, Sage View (SV) beds.  The 5 PES beds are contiguous with the ED and the 15 SV beds are on campus but stand alone and adjacent to the hospital.  We have a team of 4 FTE psychiatrists that cover the combined 20 SV/PES beds 24/7 including taking call from home.  The SV/PES psychiatric group also provide coverage for Bend based consults in the ED/inpatient floors. 

  • Work schedule: Full time, clinical position divided between CL psychiatry and ECT. The ECT portion of this FTE is expected to grow over time making up as much as 0.5 of the total FTE. This position will follow a standard M-F, 8-5pm day structure with flexibility after hours as clinical care including ECT require.
  • Call expectation:  There is no formal call associated with this position. Inpatient psychiatry will provide overnight and weekend coverage support for northern campus patients. There maybe opportunities to provide inpatient support or coverage when unforeseen absence occur within the inpatient team, but this would be voluntary.
  • Work location: CL work will be based primarily out of St. Charles Redmond, Madras and Prineville. ECT will likely occur out of Bend, Redmond or both.

ESSENTIAL FUNCTIONS AND DUTIES:

Clinical Responsibilities:

This position works within a professional team of behavioral health clinicians and learners to provide outstanding behavioral health care to adult patients in SCHS Hospitals. The team members include Psychiatrists, Qualified Mental Health Professionals, Psychiatric Residents, and UME students. Primary clinical responsibilities include:

  • Provides consults on primary psychiatric patients and for patients with a psychiatric co-morbidity.

  • Provides emergency and medical bed psychiatric consult services with the medical model of care.

  • Proactively assess high risk patients

  • Assesses and works collaboratively with ED/medical providers and care teams on an appropriate plan of care.

  • Provides psychiatric medication management, where appropriate

  • Provides education and assistance with assessment, care planning, treatment, and discharge planning to the care team through psychiatric consultation.

  • Develops individualized treatment plans based on the assessment findings in collaboration with the care team.

  • Monitors patients progress and modifies care plans as needed.

  • Participates in teaching of students, residents and fellows as needed.

  • Practices and role models excellence in the Psychiatry domains of psychiatric assessment, diagnoses, medical-psychiatric evaluation, and treatment.  To include application of clinical skill sets described above to assess acute psychiatric crisis situations such as management of agitated or violent patients, those at high risk harming themselves or others and those qualifying for intoxication or other holds or lacking capacity.  

  • Understands the application of OAR’s and pertinent regulation in psychiatric crisis situations such as OAR’s related to involuntary care.

  • Educates, counsels, and assists patients and families regarding current health problems, health maintenance activities and preventive behaviors.

  • Documents clinical services in timely manner and in keeping with clinical and financial standards.  

  • Practices medicine within the scope of license, training, and credentialing.  Cooperate with other SCHS physicians and caregivers to insure optimal patient care.

General Administrative:

  • Attends PAT meetings and regular check ins with PAT leadership to improve the quality, safety, and value of the BH care provided at St. Charles

  • Develops subject matter expertise in ECT and assists with program development, implementation, and sustainment.

  • Completes annual fire/safety, and all education in-services as required.

  • Attends mandatory educational offerings as described in SCHS policies/procedures and/or as required by other regulatory bodies.

  • Completes annual CME as required to maintain license and appropriate Board certification.

  • Demonstrates ability as an educator, clinician, and leader.

  • Completes in a timely manner assigned Computer Based Learning modules. 

  • Demonstrates excellent interpersonal and communication skills.

  • Be eligible for unrestricted medical licensure in the State of Oregon prior to the start of appointment.

  • Demonstrates strong team building skills.

  • Provides demonstrated participation in recent scholarly activity such as peer reviewed funding, publication of original research or review articles in peer-reviewed journals, chapters in textbooks, publication or presentation of case reports or clinical series at scientific society meetings, or participation in national committees or educations organizations.

LICENSURE/CERTIFICATION/REGISTRATION

Required:

  • Oregon license as MD/DO

  • Board certified by the American Board of Psychiatry and Neurology or have specialty qualifications acceptable to the Psychiatry Review Committee of the Accreditation Council of Graduate Medical Education.

  • Valid driver’s license and the ability to meet SCHS driving requirements.

  • Maintain unrestricted DEA number.       

  • Maintain unrestricted ability to participate in Medicare / Medicaid, or other federal or state governmental health care programs.

EXPERIENCE

Required:       

  • Graduation from an accredited medical school.

  • Successful completion of an accredited residency program in Physician’s chosen specialty (ies).

  • Successful completion of additional educational programs necessary to enable Physician to acquire and maintain privileges to perform the range of services contemplated. This includes board eligibility/certification in the specialty being practiced.

  • Candidate should have training and/or experience in psychosomatic medicine; demonstrate strong clinical skills in patient service and demonstrate interest in the teaching and supervision of medical students and residents.

  • Experience in hospital and outpatient practice environments in Psychiatry

  • Advanced written and verbal communication skills to effectively interact with a diverse population and professionally represent St. Charles Health System

  • Experience working in a team environment and proven ability to collaborate, manage facilitation and consensus building among health care professionals and agencies to achieve expected results.

Preferred:

  • Two or more years of consult liaison and or inpatient adult psychiatry service

  • Experience with administration of ECT

  • Epic EMR software experience

PERSONAL PROTECTIVE EQUIPMENT

  • Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.
