Summary:

Serves as Diabetes and Nutrition Educator for Diabetes and Nutrition Center through planning, implementing, and evaluating the delivery and quality of education provided to both health center patients, frontline staff, and the broader community. Serves as a mentor, role model, consultant, facilitator, and educator.

Responsibilities:

Collaborates with the healthcare team to integrate nutrition therapy into the management of diabetic patients' care.Supports initiatives to decrease avoidable hospital utilizationDevelops and provides individualized diabetes and nutrition education to patients and their families and caregivers.Develops and implements educational activities for staff and the community.Collaborates with key stakeholders to address social determinants of health and strengthen integration and linkages by wrapping community health programs and services around vulnerable populations.Assesses patient understanding and cooperation with integrating nutritional planning into the patient's lifestyle. Modifies plans to promote success.Participates in the required activities to maintain ADA Certification.Serves as a resource for front line staff and supports education deficits.

Required Work Experience: Two (2) years' relevant work experience pertinent to Diabetes Self- Management Education and Support Training (DSMEST).Experience with program planning and evaluation (i.e., pre- and post-test, repeat demonstration, clinical measures, evaluating trends, and communicating plan of care).
