Summary :

The Structural Heart Program Specialist assists in ensuring a quality-based and patient-focused program that is devoted to appropriate management of the population with valvular heart disease. Implements key protocols and processes for managing referrals, communicating patient information to all providers, scheduling appropriate evaluations and follow-ups, and managing the database to track relevant valve clinic patient information and activity. Coordinates patient flow from presentation clinically to the heart team review, to scheduled visits, to clinical testing and evaluation, and procedure setup. Coordinates work with the patient.

Job Description:

KEY ACCOUNTABILITIES:

  • Collaborates with the cardiovascular and thoracic surgery team to assist with the individualized assessment, planning, and evaluation of patient care by providing independent and delegated services to patients and significant others, which integrates cultural diversity and special needs appropriate to the patient population served.

  • Coordinates with primary care provider, cardiology, and cardiothoracic surgeon to facilitate appropriate patient follow up and determination of care based on the American College of Cardiology (ACC)/American Heart Association (AHA) guidelines for follow-up echocardiography or advancement.

  • Completes documentation for the structural heart program to meet clinical quality standards.

  • Teaches others about structural heart program to meet educational needs both internally and externally. Designs and coordinates educational materials used.

  • Addresses situations or decisions to ensure patient concerns or needs are met. Demonstrates professional and therapeutic relationship with patients, families, and providers.

QUALIFICATIONS:

  • Bachelor’s degree in nursing

  • CPR and ACLS

  • Current State of Wisconsin registered nurse license

PHYSICAL DEMANDS:

  • Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of fifty (50) pounds without assistance

  • Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties

WORK ENVIRONMENT:

  • Frequent exposure to sharp objects and instruments

  • Occasional exposure to moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electrical shock

  • Occasional high noise level in work environment

  • Possible exposure to communicable diseases, hazardous materials, and pharmacological agents

  • Occasional contact with aggressive and or combative patients
