JOB DESCRIPTION:

SUMMARY OF POSITION

Jump start your career in the mental health field as a Case Manager. You’ll coordinate services and care for patients – providing coaching, strategies and coordination of health and wellness resources so they can live their lives more fully. Through your work, you will teach people the importance of the connection between physical and mental health.

We are looking for people who are ready to help make a difference in the lives of the people we serve every day. The mental health needs in our community have never been greater. If you have a bachelor’s degree in counseling, psychology, social work or related field – and you’re passionate about making a difference in the lives of people coping with mental health issues, we need you!

You will become a trusted resource for your patients, offering connection and education about how recovery works best when we address the whole person.

You will enjoy the flexibility of working in a variety of settings – the clinic, patient homes and in the community. This is a great opportunity if you’ve just graduated or if you’ve had a little experience but you’re ready for more.

 ESSENTIAL DUTIES & RESPONSIBILITIES

  • Help patients navigate the health care system through coaching, education and support.
  • Coordinate the behavioral and primary care needs and develop strategies to remove barriers to wellness.
  • Use behavioral management techniques, crisis intervention, active listening and motivational interviewing to help patients develop better life management skills.
  • Design and implement individually tailored service plans.
  • Demonstrate knowledge and understanding of ethical standards by evidence of practice with patients and community partners.
  • Attend weekly staffing and team meetings and completes assessments.

KNOWLEDGE, SKILLS & ABILITIES

QUALIFICATIONS

         

Education Level              

Bachelor’s degree in psychology, social work or related field

Years of Experience       

  • Experience with Electronic Health Record preferred, but not required

Certification/Licensure 

Valid driver’s license in good standing required.

$19.10--$26.80
