Overview

The Clinical Manager for Gold Brown Residence (GBR) and Preceptor Program oversees the daily operations of the facility, ensuring high-quality patient care in a compassionate and supportive environment. Manages nursing staff, coordinates patient care, and collaborates with interdisciplinary teams to provide comfort and support to patients and their families during end-of-life care. Serves as a preceptor to new hires, including developing a new grad RN/LPN residency program. This role will require full-time presence in GBR.

Compensation Range:$98,200.00 - $130,800.00 Annual

• Supervises home health aides, as required per regulatory bodies.
• Responsible for preceptorship of nursing staff, both RN and LPN’s for the Residence and Field positions.
• Collaborates with Education team in developing and implementing a new nurse residency program at the Goodman Brown Residence (GBR).
• Ensure the delivery of high-quality, patient-centered care in accordance with hospice standards and regulations.
• Assures quality care metrics are met for the Goodman Brown Residence
• Facilitates family meetings, in conjunction with Social Work team to discuss patient care, treatment options, and end-of-life decisions.
• Works closely with physicians, social workers, spiritual care providers and volunteers and other healthcare professionals to develop and implement comprehensive care plans.
• Participates in quality assurance initiatives to enhance care delivery and patient satisfaction.
• Implements evidence-based practices and stay current with hospice care trends and regulations.
• Ensures adherence to all relevant policies, procedures, and regulatory requirements.
• Acts as resource for clarification of documentation and use of the tablet.
• Provides feedback to the Director of After Hours and Residence on the development and management of the budget for the hospice residence.
• Monitors resource utilization to ensure efficient and effective care delivery.
• Participates in special projects and performs other duties as assigned.

Qualifications

Licenses and Certifications:
Certified Hospice and Palliative Nurse (CHPN) Certification required and
Wound Care Certification (WCC) required
Current license to practice as a Registered Nurse in New York State  required

Education:
Bachelor's Degree in Nursing required

Work Experience:
Minimum of seven (7) years’ experience in health care with a minimum of five (5) years of focus in Hospice required
Previous preceptorship or education experience in hospice preferred preferred
Knowledge of HCHB operating system for hospice required
Knowledge of other community based software systems such as SWIFT, HIPAA compliant messaging preferred
Strong clinical, communication and interpersonal skills required
Excellent critical thinking and problem-solving abilities required
Compassionate and empathetic approach to patient care required
Superuser of Home Care Home Base (HCHB) preferred
