Job Description:
SCOPE: The Oral Nurse Navigator will work with a multidisciplinary team consisting of triage nurses, schedulers, oncologists, and pharmacists to provide highly coordinated care and to identify and eliminate barriers. Workflows include following the oral triage internal and external script process; prescription review; investigation of benefits; routing script to appropriate pharmacy; financial assistance; scheduling oral chemotherapy teaching with Nurse Practitioner; monitoring patient adherence; educate and update patient on the process of receiving the medication related to delivery, timeline, finance, and the start of therapy. In addition, the Oral Nurse Navigator will serve as a point of contact for oral chemotherapy for the Metairie area. An ideal candidate should be able to assess and prioritize patient needs, have good time management, pay strict attention to details, excellent documentation, and must be able to work independently, as well as, work well with others.
FUNCTIONS:
1. Coordination of the oral chemotherapy process; following oral triage internal and external workflow; identify and breaking down barriers; Provide patient and family education on process, improve timeliness of care; Collaborate with care team through multi-disciplinary communication; patient education; adherence and compliance monitoring. Ensure adherence to legal requirements, government reporting regulations, QOPI, etc. as they pertain to the clinical oncology processes.
2. Maintains ongoing relationship with patient to ensure oral chemo adherence, provide education on side effect management, safe handling and disposal of oral chemotherapy. Acts as a liaison between patient, clinic, retail pharmacy, and physician. Educate patient on the importance of oral chemo adherence.
3. Monitoring patient adherence to follow up schedule. Reinforce knowledge of schedule with patient/family. Schedule initial oral teaching and follow up adherence appointments; Responsible to follow up with patient for no shows and cancels. Serves as point of contact for oral chemotherapy for the Greater Baton Rouge area.
4. Apply system-thinking, problem-solving, and collaboration skills. Proficient in the use of EMR and instill a high degree of confidence through education, support, and flexibility. Be available to address operational problems and promote a strong and cohesive team. Assist in development of EMR policy and processes to reflect QOPI standards and improve patient care
QUALIFICATIONS:
Associates degree in nursing required; BSN preferred.
Active RN or LPN license in good standing with the Louisiana State Board of Nursing.
Chemotherapy and Immunotherapy Certification required (ONS) or obtained within 30 days of hire; OCN Certification preferred.
Experience in Oncology or Infusion nursing highly preferred.
