Keeping patients connected to the highest quality care as they transition from the hospital to home or a facility is part of the Cleveland Clinic innovative approach to value-based care. As a Hospice Coordinator, you will be at their side the whole way, ensuring a smooth adjustment to their new routine. Through our Center for Connected Care, you will help provide a full continuum of care by processing inpatient referrals, coordinating discharge plans, coordinating services and more. Your efforts directly enhance the quality of life for patients and their families every day. This position will support our southern region – Akron General, Medina, Lodi and Mercy.
A caregiver in this role will work three 12-hour shifts per week from 8:00am to 8:30pm, with all worked hours performed on-site within the hospitals (covering Akron General, Medina, Mercy and Lodi Hospitals). This position includes an every-fourth-weekend rotation and a holiday requirement of two holidays per year, including one summer holiday and one winter holiday.
