Job Summary

Maintains accountability for ensuring the safety and efficacy of medication use. Educates and acts as a consultant to patients/family and members of the health care team related to optimal drug therapy and participates in process improvement activities and drug use evaluations. Works with multiple medication automation systems.
 

Essential Functions

  • Interprets and screens medication orders for appropriate agent, dose, route, allergies, and potential drug interactions. Safely compounds and dispenses medications for administration to patients.
  • Evaluates patients, orders appropriate laboratory tests, and interprets serum drug levels utilizing pharmacokinetic principles; provides prescribers with dosage regimen recommendations to optimize therapeutic outcomes, reduce patient risk, and monitor the costs associated with drug therapy.
  • Provides appropriate supervision and direction within appropriate skill level to technicians, residents, interns, and other support personnel.
  • Engages in continuous study of the entire professional field to maintain the professional competence, knowledge, and skills necessary for satisfactory performance of all assigned responsibilities. Completes required continuous training and education, including department and Spectrum Health specific requirements on a timely basis.
  • Provides education and counseling to patients/family/significant others/members of the health care team in order to optimize outcomes and appropriate use of medications.
  • Ensures that services are provided in accordance with State and Federal regulations, organizational policy, and accreditation/compliance requirements.

Qualifications

  • Required Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy
  • 2 years of relevant experience hospital pharmacy Preferred
  • American Society of Health-System Pharmacists (ASHP) accredited residency in Pharmacy Practice Preferred
  • LIC-Clinical Pharmacist - STATE_MI State of Michigan 90 Days required
  • LIC-Pharmacist Controlled Substance - STATE_MI State of Michigan 90 Days required
  • CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association 180 Days required Or
  • CRT-Pediatric Adv Life Support (PALS) - AHA American Heart Association 180 Days required
  • Ideal candidates will be a Pharmacist possessing a PharmD Degree while also possessing Hospital experience, ideally in Oncology or Critical Care, or/and possessing a PGY1 Residency or a PGY2 Residency in Oncology
  • This role will provide support to the Greenville Cancer Center as well as Greenville inpatient pharmacy operations
