The diabetes care and education specialist (DCES) delivers comprehensive diabetes care services that bridge the gap and integrate clinical and self-management aspects of diabetes and cardiometabolic care. The DCES is an integral part of the interprofessional team and provides collaborate, comprehensive and person-centered care along with education that is conducive to behavior change and improved quality of life across the lifespan. The DCES supports and advocates for people affected by diabetes to optimize quality care. The DCES promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes.
Pay Range: $26.89 - $41.95/hour
Employment Type: Full Time, 80 hours every 2 weeks
Schedule: Monday - Friday 8:00 am - 4:30 pm
Requirements: Bachelors of Science in Nursing (BSN) or Bachelor of Science in Nutrition & Dietetics (RD or RDN) required, Current Michigan RN License or Registered Dietitian required, Credentialed as a Certified Diabetes Care and Education Specialist or completed within one year of hire required, RN or RD with minimum of 2-4 years recent experience in diabetes care and/or patient education required, Current BLS for the Healthcare Provider or completed according to RQI requirements following new employee orientation.
CLINICAL PRACTICE
- Applies current principles of evidence-based practice.
- Participates in the evaluation of program processes.
- Interprets physical and psychosocial data.
- Evaluates diabetes care delivery according to current standards of care.
- Assesses current trends from diabetes research for application to practice.
- Identifies risk factors for diabetes and related diseases (hypoglycemia, diabetic ketoacidosis, hyperglycemic state)
- Identifies changes in medical/mental/physical status that affect the person’s ability to follow medical treatment plan and refers to appropriate team member (BHS, PCP, etc.)
- Maintains and applies advanced clinical knowledge and skills appropriate to the educator’s scope of practice.
- Stays current on diabetes-related technologies such as continuous glucose monitors and insulin pumps and other healthcare-related technologies for data collection and analysis.
- Works with participants and the healthcare team to individualize the diabetes medication regimen and assists in determining lower medication cost opportunities.
- Follows medication adjustment protocols making medication changes in collaboration with primary care and/or endocrinology providers.
- Makes necessary medication recommendations to providers.
- Identifies local community support networks for people living with diabetes.
- Assess adequacy of daily nutrition intake and refers to RD/RDN as needed.
- Evaluates continuous glucose monitoring (CGM) records to achieve and maintain goals in high-risk person.
PATIENT EDUCATION AND TRAINING
- Collects participant health data and completes a comprehensive health assessment in a collaborative and ongoing manner utilizing motivational interviewing skills to assist in appropriate and measurable goal setting.
- Applies shared decision-making to promote behavior change.
- Collaboratively develops educational goals, learning objectives and an individualized plan with participants.
- Facilitates problem-solving to overcome barriers.
- Applies knowledge of diabetes pathophysiology to direct diabetes education and/or diabetes care.
- Provides educational interventions that utilize primarily interactive, collaborative, skill-based training methods.
- Collaboratively Develops an individualized follow-up plan with each program participant.
- Evaluates effectiveness of educational services provided by measuring attainment of learning objectives.
- Documents assessment data, educational plan, educational services provided and evaluation results in each participant's record.
COLLABORATION
- Utilizes a team approach to provide services, identifies when a participant's needs are outside of the educator's scope of practice and expertise and refers participant to other resources.
- Facilitates coordination and communication with healthcare team.
- Serves as a content expert for others related to the care of people with diabetes.
- Serves as a resource in curriculum development and maintenance, program planning, implementation, and ongoing evaluation.
- Develops algorithm or protocol-based medication adjustments for changes in meal plan or exercise.
QUALITY IMPROVEMENT
- Contributes to and participates in a continuous quality improvement process to measure the program and to identify and address opportunities for improvement.
TRAINING AND DEVELOPMENT
- Supports the professional development of diabetes educators and other healthcare professionals.
- Mentors new diabetes educators
- Develops and delivers diabetes education programs for healthcare providers.
- Facilitates training of diabetes self-management education and support (DSMES) program staff and primary care provider (PCP) offices on how to interpret annual labs, testing to assess current status of disease progression and needs for additional medical/psycho-social interventions.
PROFESSIONAL REQUIREMENTS
Bachelors of Science in Nursing (BSN) or Bachelor of Science in Nutrition & Dietetics (RD or RDN) required
Current Michigan RN License or Registered Dietitian required
Credentialed as a Certified Diabetes Care and Education Specialist or completed within one year of hire required
