Nurse Care Manager
Behavioral Health Homes
The position of Nurse Care Manager is a member of the Behavioral Health Home (BHH) Adult Service Team and the BHH Children’s Service Team. In both BHH programs, the Nurse Care Manager is to provide nursing consultation to the BHH Care Coordinators and Peer/Parent Navigators, oversight of BHH health outcomes, and collaboration with BHH Primary Care Partners. It involves health data interpretation and reporting; clinical problem solving; and collaborating with an integrated health and wellness service team serving adults and children with serious and persistent mental illness. It supports recovery-oriented service that strives to improve health outcomes through health promotion & education and primary, secondary, and tertiary prevention; cost effectiveness; and improved consumer-provider partnership for the population served by the BHH.
This role fosters highly skilled communication between behavioral health staff, peer/parent navigators, primary care partners, MaineCare staff and other members of the client’s care team in the interest of consumers and families served at the BHH and CBHH.
Schedule: Full-time, salaried role, M – F, regular business hours.
Location: on-site in South Portland, ME
