Job Description :

Your Job: In this highly technical, fast-paced, and challenging position, you'll collaborate with multidisciplinary team members to provide the very best care for our patients. The Clinical Outcomes Coordinator primary function is the assessment, analysis, and management of clinical outcomes for patient populations. Responsibilities include, but are not limited to, daily case review/monitoring, chart review, data management, process improvement and education. Your Job Requirements: • Bachelor's degree in nursing, healthcare or related field required • Master of Science, (Waiver may be granted for equivalent experience, knowledge, and skills with Director and Assistant Executive Director approval) • Current license to practice professional nursing in Texas • License from respective agency for non-nurse professional • 5 years of health care experience; prefer 3 years in quality or performance improvement arena • If non-certified in specialty must obtain certification within three years Your Job Responsibilities: • Communicate clearly and openly • Build relationships to promote a collaborative environment • Be accountable for your performance • Always look for ways to improve the patient experience • Take initiative for your professional growth • Be engaged and eager to build a winning team
