The LPN Teleheath Case Manager (CM) is responsible for the day to day operations of the Telehealth Program. The Telehealth CM will manage patients remotely, coordinating care and contacting physicians as appropriate. They support the PR Quality role and telehealth only services billing. They are an integral part of preventing re-hospitalizations and Emergency Room visits for patients, facilitating care in the community through communication with physician offices, community resources, patients and families.

Pay Range:  $23.31 - $36.36/hour

Employment Type:  Part Time, 40 hours every 2 weeks

Schedule: 8:00 am - 4:30 pm on various days, no weekends, no holidays

Requirements:  Graduate of an accredited LPN program required, Michigan LPN license, BLS for the Healthcare Provider within 60 days of hire

PRIVATE DUTY TELEHEALTH PATIENT OVERSIGHT

• Performs daily clinical oversight of all PR tele-health patients, including monitoring trends, reporting to physicians and coordinating follow up care as needed.
• Advocates for interventions to prevent re-hospitalizations and Emergency Room visits. Identifies and helps facilitate identification of telehealth appropriate patients.
• Educates patients and ED and inpatient care managers and home health staff about telehealth only services.

PRIVATE DUTY QUALITY & BILLING SUPPORT

• Supports the PR Quality Role and billing preparation.
• Provides timely, fair, and unbiased performance feedback to Telehealth Team Lead for use in performance evaluations.
• Assists the Telehealth Team Lead with developing and implementing new processes to improve patient care, improve effectiveness and efficiency of telehealth referrals, and day to day operations of private duty telehealth within Home Health services.
