Job Summary

The Ambulatory Care Pharmacist provides clinical services for medication management in the ambulatory setting. The pharmacist will interact with physicians and advanced practice providers under a collaborative practice agreement to maximize therapeutic medication benefit for select patients.

Job Description

Essential Functions:

  • Provides direct patient care under a collaborative practice agreement to establish patient-specific therapeutic medication regimens.
  • Answers clinical questions, as requested by clinic providers.
  • Assists clinic staff as needed with medication access, including prior authorizations, appeals, patient enrollment in manufacturer assistance programs, third-party grants, and copay assistance programs.
  • Provides medication education to the healthcare team, patients, and community.
  • Contributes to guideline or policy development and implementation.
  • Conducts quality improvement initiatives and practice advancement projects.
  • Precepts PGY1 and PGY2 Pharmacy Residents and pharmacy students.
  • All other duties as assigned.

Anticoagulation Clinic:

  • The Anticoagulation Pharmacist working in the Ambulatory Clinic setting manages the anticoagulation needs of patients assigned to the clinic.  The pharmacist may also assist with other cardiovascular conditions, including hypertension, dyslipidemia, cardiomyopathies, cardiovascular risk reduction, and heart failure therapy by being available to answer clinical questions and assist with medication access.

Additional Requirements

Required:

Education - Bachelors in Pharmacy

Certifications - Pharmacist - South Dakota Board of Pharmacy; Basic Life Support (BLS) Certification - American Heart Association (AHA)

Preferred:

Education - Doctorate in Pharmacy

Experience:

  • Completion of a PGY1 pharmacy practice residency plus a minimum of three years of clinical pharmacy experience in the ambulatory care setting OR
  • Completion of an Ambulatory Care PGY2 pharmacy practice residency (or specific focus area, such as cardiology) OR
  • Five years of clinical pharmacy experience in the ambulatory care setting

Certification - Board certification is encouraged

Light work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
