This role is designed to serve the community by decreasing disparities through navigation and education. The candidate in this role will build working relationships, solve problems, remove barriers to care and support patients while assisting them through the continuum of care.  In addition, you will educate people within the community on cancer disease, prevention, and necessary cancer screenings. This position requires someone who can coordinate appointments and testing for early cancer detection and assisting with removing barriers to care, such as insurance and financial needs, transportation, knowledge and other psychosocial concerns. The Community Outreach Patient Navigator identifies and links eligible patients and their families to required screenings and services. 

The Community Outreach Patient Navigator  role will require travel to other locations and sites based on event needs. This may require weekend or evening work for events.

A caregiver who excels in this role will:  

  • Coordinates and educates patients within the community on cancer, cancer prevention and screening

  • Identifies and addresses barriers to care, fear, trust, financial, insurance, transportation and psychosocial to ensure appropriate clinical care is coordinated and provided in a timely manner

  • Help patients navigate our complex health care systems

  • Coordinates appointments related to cancer screenings i.e. mammogram results, colonoscopy results and pathology reports

  • Navigates patients to appropriate health care provider  within all Cleveland area  facilities

  • Identifies barriers to care  and coordinates  resolution to including but not limited to

-Social work support

                 - Medicaid and Medicare

                   -Prescription Assistance Programs

                   - Community resources (gathering place & 211)

                  - Connecting patients to primary care and medical homes

  • Support coordination and execution of  community events 

Minimum qualifications for the ideal future caregiver include:    

  • High school diploma required.

  • Minimum three years related experience in customer relations with diverse patient/culture populations, exhibiting problem solving skills.

  • Excellent interpersonal skills with special emphasis on oral and written communication, process improvement, and problem resolution.

  • Requires strong communication, analytical, and critical thinking skills.

Preferred qualifications for the ideal future caregiver include:  

  • Bilingual in Spanish preferred.

Languages:

  • Spanish (orally and written) preferred

Competencies (Complexity of Work):

  • Knowledge of Electronic Medical Record (EMR) preferred

  • Ability to deal effectively with a variety of people and work in a team environment

  • Excellent patient service orientation skills

  • Must possess strong oral, written and organizational skills
