About the Role
The Polio Service Victoria (PSV) team is seeking a Rehabilitation Physician to join our team on a fixed term basis to cover LSL from 1/9/2025  until 28/11/2025.
 
You will work within a flexible, interdisciplinary structure, that ensures client’s  needs are met to receive ‘the right treatment in the right place at the right time’.  The rehabilitation physician provides medical leadership in the management of the late effects of polio and works with the multidisciplinary team within clinic appointments and via case conferencing to develop a management and monitoring strategy for patient care.
 
A unique feature of PSV is delivery of multi-disciplinary regional clinics 6 times per year.  During this tenure, you would ideally be available to attend one overnight regional clinic with the interdisciplinary team in October 2025.  (Flexible working arrangements for this aspect may be negotiated, i.e. Rehabilitation Physician attendance by telehealth will be considered.) 
 
The role is otherwise based at St Vincent’s Hospital, Fitzroy. This position is up to 12 hours per fortnight (or by negotiation) with Wednesday afternoons and full days on Thursdays - alternating weeks.

Classifications for this position will range from HN16 - HN56 ($160.20 - $208.45 per hour dependent on years of experience).

Your Contribution
• Specialist registration with the Medical Practitioners Board of Australia / AHPRA
•  Fellowship of the Australasian Faculty of Rehabilitation Medicine or equivalent specialist qualification
•  Participation in an approved CME program
•  Experience in management of people with a history of polio is preferred
• Current National criminal history check, or willing to obtain/ Current NDIS Worker Screening Check, or willing to obtain (Select which is applicable)

About the Department
The Health Independence Program (HIP) was introduced by the DH&HS in 2008 bringing together the four previously separate service streams of Subacute Ambulatory Care, Post Acute Care, Hospital Admission Risk Program and Residential In Reach under common guidelines. HIP consists of integrated hub sites located across several locations including in Fitzroy, Kew, North Richmond and Fairfield. The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning. This position may be required to work at any of the HIP hub sites / programs. This will be at the discretion of the HIP manager and will take into consideration appropriate skills and training.
