Job Description:

Behavioral Health Registered Nurse Short Visits

Position: Full-Time
Schedule: Days with occasional afternoon visits as needed
Coverage Areas: Norwalk & Stamford, CT

Responsibilities

As a Behavioral Health Registered Nurse, you will:

  • Conduct assessments to identify the physical, psychosocial, and environmental needs of patients.
  • Take primary responsibility for a caseload, including planning, coordinating, and evaluating treatment plans.
  • Perform initial evaluations and ongoing nursing assessments during each visit.
  • Communicate significant health findings and collaborate with clinical supervisors, physicians, and care teams.
  • Revise care plans in response to patient needs and provide education to patients and families.
  • Supervise and document the care provided by behavioral health aides and LPNs.
  • Support patients and families during end-of-life care and perform bereavement assessments as needed.
  • Educate staff and caregivers on behavioral health philosophy and care management.

Qualifications

  • Licensure: Current, unrestricted RN license in Connecticut.
  • Experience: Minimum of 1 year in a clinical care setting; home health or hospice experience is a plus.
  • Education: Associate’s or Bachelor’s degree in Nursing (preferred).
  • Travel: Willingness to travel up to 50% within the coverage area with a dependable vehicle, valid driver’s license, and current auto insurance.
  • Technical Skills: Experience with Homecare HomeBase is preferred.
