The Pathways Program aims to increase the capacity of healthcare providers and staff to recognize and respond to the needs of our patients impacted by domestic violence, human trafficking and sexual violence. In partnership with the IL Department of Human Services, Pathways will be establishing a trauma informed multi-disciplinary primary care clinic for survivors of gender-based violence and their children.

This is a temporary grant-funded position – should financial funding for this role be discontinued at any time or cease beyond the duration of the grant period, the position will no longer remain active.

What you will do:

The Pathways Program aims to increase the capacity of healthcare providers and staff to recognize and respond to the needs of our patients impacted by domestic violence, human trafficking and sexual violence. In partnership with the IL Department of Human Services, Pathways will be establishing a trauma informed multi-disciplinary primary care clinic for survivors of gender-based violence and their children. This position will work closely with a multi-disciplinary team of clinical staff and victim services to improve patient health outcomes.

Responsibilities and Functions

  • Provide direct support including bedside crisis intervention, safety planning and assessment of needs to patients impacted by domestic violence, sexual violence and human trafficking.

  • Provide ongoing case management, legal advocacy, information and referrals to our patients and staff impacted by gender-based violence.

  • Provide medical advocacy and healthcare navigation for referrals received by our partner victim-service organizations and manage internal processes for billing, scheduling and referral tracking for these patients.

  • Provide coverage to internal crisis line that providers and staff use to refer patients and respond to those immediate requests through bedside crisis intervention or follow-up calls with patients who have been discharged.

  • Complete all necessary documentation in internal program database and document patient assessments in electronic medical record.

  • Support professional development initiatives for providers throughout the hospital and outpatient practices on the subjects of providing trauma-informed care and working with patients impacted by gender-based violence.

  • Coordinate awareness-raising events on hospital campus and in the local community to support people impacted by domestic violence, human trafficking and sexual assault.

  • Assist with preparation and attendance of internal and external meetings and represent the program at public speaking and training events.

  • Other projects and duties as assigned

Minimum Requirements

  • Education: Bachelor’s Degree

  • Certification: 40-Hour Domestic Violence Training or 40- Hour Sexual Assault Crisis Intervention Training

Knowledge, Skills and Abilities (KSAs)

  • Preferred fluency in Spanish

  • Exceptional executive functioning skills and emotional intelligence

  • Ability to take initiative and work independently, as well as in a team environment that includes cross departmental coordination

  • Strong passion, leadership and interpersonal skills necessary to effectively work with multi-cultural, diverse populations from a trauma-informed lens

  • Commitment to health equity and survivor-centered practices

Environment

Light to moderate physical effort is required. May require frequent periods of standing, walking, kneeling, and bending.  Requires occasional lifting of lightweight objects.  Employees must ask for help in the rare lifting of heavy weight materials. Potential exposure to airborne diseases.
