Job Description​

We are hiring an RN to join our Hospital In Your Home Team at the Plaza.

The Hospital at Home virtual RN is a member of a collaborative care team who is responsible for providing tele-nursing care to a diverse population of patients. The following features govern the role: patient advocacy, prevention, telecommunication, proactive continuous monitoring, evidence-based care, clinical decision support and humane, compassionate, and ethical care.

This is a PRN dayshift position. Coverage is 7 days per week.

This will be an experienced nurse that monitors assigned patients using intelligent software. They will be alerted to any changes in patient condition or trending that needs to be addressed.

  • Monitoring intelligent software and vitals for changes and trends or patient condition.

  • Good communication skills in tense situations.

  • Have years of experience to understand what they are looking for in the trending data.

  • Early detection of changes or possible problems.

  • In-patient experience required. 

  • Must have excellent verbal and written communication skills and possess strong computer skills.

  • A minimum of 3 years of experience preferred. 

  • Experience providing care in a patient’s home or via telehealth/remote monitoring a plus.

  • Flexible and comfortable with a changing and dynamic work environment and schedule across all shifts and days  

  • Functions independently and accountable for utilizing the nursing process.

  • Demonstrates increasing discriminatory judgement in interpreting data and prioritizing appropriately.

  • Initiates actions to improve nursing practice and patient care and unit processes.

  • Serves as a day-to-day resource to colleagues at unit and department levels.

  • Utilizes appropriate hospital resources.
