Your Impact as a Registered Nurse Behavioral Health:

  • Patient Assessment: Evaluate the physical, psychosocial, and environmental needs of your behavioral health patients and their families, ensuring comprehensive care plans are in place.
  • Case Management: Take primary responsibility for your caseload, from assessment to planning, coordinating, and evaluating care plans.
  • Holistic Care: Incorporate physical, psychological, social, and spiritual factors into your care plans, addressing all aspects of patient well-being.
  • Ongoing Evaluation: Perform initial and follow-up evaluations to adapt care plans based on patient, family, and facility needs.
  • Effective Communication: Share significant findings, health changes, and safety concerns with the Clinical Supervisor, physician, and other team members to ensure coordinated care.
  • Care Plan Adjustment: Revise care plans as needed and keep all stakeholders informed of changes.
  • Patient & Family Education: Guide patients, families, and facilities through the Interdisciplinary Plan of Care, teaching behavioral health philosophy and symptom management.
  • Supervision & Documentation: Oversee behavioral health aides and LPNs, ensuring care is delivered according to agency policies and documenting all aspects of care.
  • Facility Coordination: Inform supervisors of any unusual or problematic issues within care facilities.

What is Required?

  • Current, unrestricted state RN licensee
  • 1+ years of experience in a clinical care setting as a nurse
  • Able and willing to travel within branch/office coverage area.
  • Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
  • Must be able and willing to travel 50%
  • Associates Degree or Bachelors Degree in Nursing is preferred
  • Previous Home Health or Hospice Experience is preferred
  • Previous experience with Homecare HomeBase is preferred
