Title: Transitional Care Coordinator

Role Description:

We take a team-based approach to providing outstanding patient care, and Transitional Care Coordinators are an integral part of that team. In this role, you will help track patients across hospital and post-acute settings and ensure they reconnect with primary care. You will manage multiple responsibilities, including outbound phone calls to facilities and patients, handling medical records, and scheduling post-discharge appointments. Flexibility is key, as you may be asked to cover additional responsibilities as needed. This is a remote work position.

Core Responsibilities:

  • Manage inbound notifications related to transitional care events from payers, vendors, facilities, and other sources

  • Conduct outreach to inpatient and post-acute facilities to obtain admission and discharge dates, as well as discharge plans

  • Request and obtain medical records, route documents to clinical staff for review, and upload documents into the electronic medical record platform

  • Conduct outreach to patients to schedule and confirm post-discharge appointments within the electronic medical records platform

  • Coordinate with care teams, facilities, payers, and community-based organizations to ensure patients’ transitional care needs are met

  • Perform other duties as assigned

What We’re Looking For:

Required Qualifications:

  • High school diploma

  • Typically less than 1 year of relevant experience

  • Strong computer skills, including basic knowledge of Microsoft Excel and electronic medical records

  • Experience in a customer service setting (healthcare experience preferred but not required)

  • Professional phone etiquette

  • High level of integrity

  • Proficient PC skills

Preferred Qualifications:

  • College degree

  • Fluency in Spanish, Polish, Russian, or other languages spoken within the communities we serve

Education:

  • High school diploma required; college degree preferred

Pay Range

The typical pay range for this role is:

$17.00 - $31.30

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
