Diabetes Coordinator/Educator - SOM-Peds-Endocrinology

Job Summary:

To provide education to patients and families that promotes diabetes self-management in the inpatient and ambulatory settings. To assess, develop, implement, coordinate, and evaluate specified educational/instructional programs provided to staff, patients, and/or family members as applicable to the position. To develop and implement strategic needs analyses and training plans. To perform job duties in accordance with the medical center's purpose.

Education & Experience

Valid Mississippi RN license or BS degree in nutrition and dietetics, and two years diabetes related experience.  Recent experience preferred. Experience in program development and management recommended.

Certifications, Licenses or Registration required:  Valid Mississippi RN license or current Mississippi professional licensure as a Registered Dietitian (RD), within 6 months of hire, must be board Certified Advanced Diabetes Management (BC-ADM) or Certified Diabetes Educator (CDE), and have certification and maintenance on various insulin pumps - and insulin adjustments on non-pump patients.

Knowledge, Skills & Abilities

Ability to use blood glucose meters, insulin infusion pumps and other office and clinic equipment.  Ability to communicate effectively in group and individual education for patients and their families.  Ability to develop and provide education materials for staff and community.  Ability to implement proper protocols for diabetes patients.  Interpersonal skills, communication, and customer service skills. Knowledge of diabetes, pediatric and adult learning principles and age-appropriate instructional techniques.


Responsibilities

  • Oversees the planning, implementation and evaluation of the DSME program. Responsible for assessing, organizing, coordinating, implementing, instructing, and evaluating specific outpatient educational programs related to diabetes including providing comprehensive diabetes care and education to pediatric patients and their families with diabetes and for working cooperatively in all areas of the diabetes program providing education and diabetes care in group and individual sessions.
  • Arranges and coordinates the activities of the advisory group.
  • Responsible for maintaining ADA recognition and participating in the evaluation of the program's effectiveness.
  • Liaises between the staff, the advisory group, other departments and administration. Collaborates with all members of diabetes center team in effort to improve outcomes of patients.
  • Participates in recruitment of research subjects via various avenues; chart review, written correspondence, community outreach, lectures, workshops. Attends community health fairs and supervises events with diabetes camps and other diabetes forums to promote clinical research activities as well as diabetes awareness and education
  • Evaluates the effectiveness of instructional, training and development programs and utilizes relevant evaluation data to revise or recommend changes in instructional objectives, methods, and/or courses.
  • Collaborates with all members of diabetes center team in effort to improve outcomes of patients.
  • Prepares and submits monthly reports detailing teaching educational, and professional activities; maintains applicable records and files.
  • Demonstrates positive interpersonal skills, communication, & customer service skills
  • Teaches child/family skills for diabetes self-management.  Maintains professional development of self and the development of peers.
  • Participates in research to enhance practice.
