What You’ll Do:
• Provide comprehensive medical nutrition therapy, including assessments, individualized care plans, diet modifications, and ongoing evaluation to support healing and long-term wellness.
• Collaborate with physicians, nurses, and interdisciplinary teams to ensure nutrition interventions align with clinical goals and NWH’s high standards of care.
• Document thoroughly and accurately, including assessments, recommendations, care plans, follow up data, and all relevant clinical activities to support continuity and quality of care.
• Educate and empower patients and families with clear, compassionate guidance on therapeutic diets and nutrition strategies that support recovery and lifestyle change.
• Partner with Food & Nutrition Services to ensure meals, supplements, and nutrition support are delivered accurately; review menu selections to support adherence to modified diets.
• Support safe transitions of care, including discharge planning and coordination of ongoing nutrition needs.
