Job Description Summary:

Overview

Responsible for the care and service of selected patient populations requiring home infusion services from neonates to geriatrics. Promotes effective utilization and monitoring of healthcare resources, and assuming a leadership role with the interdisciplinary team to achieve desired clinical and resource outcomes. Current RN licensure as approved for practice in the State of Tennessee, current driver's license, CPR certification required. Experience in administering IV therapies required and managerial skills preferred. The employee is subject to call back and overtime as required by the hospital.

Responsibilities

Assess on a concurrent basis for the appropriateness of the level of care, diagnostics and procedures performed, quality/clinical risk issues, and documentation of patient record completeness, reports all variances according to the established process.
Communicates and coordinates communication with pharmacists, patients, families, medical staff, caregivers, and third-party payors according to the plan of care.
Assess, develop, implement, and monitor a comprehensive plan of care through an interdisciplinary team process in conjunction with the patient family in internal and external settings.
As part of the Quality Outcomes Improvement process, participates, and/or coordinates the monitoring of outcome data.
Monitors and coordinates the monitoring of the patient's care.
Evaluates the outcomes measured by the Quality Outcomes Improvement program.
Documents care in the patient's record.
Formulates, implements, and evaluates educational strategies for patients and families.
Educates patients/families and evaluates education.
Troubleshoot problems with catheters and pumps in coordination with pharmacists.
Ability to provide technical expertise in the area of intravenous therapy.
Performs related responsibilities as required or directed.
Ensures safe delivery of home infusion services and products to patients serviced by MCIS and documents the care provided.
Provides in-service education to healthcare professionals, as needed.
Coordinates the Quality Outcomes Improvement program and activities in a manner that reflects company philosophy, as well as the accreditation and other standards of home infusion services.

Qualifications

EDUCATION: Skill and proficiency in applying highly technical principles, concepts, and techniques that are central to the nursing profession. Such proficiency is acquired through the completion of a diploma program, Associate or Baccalaureate degree program in Nursing. Knowledge of age-specific needs of growth and development for assessment, range of treatment, and care of patients, as acquired through formal educational coursework. Knowledge of JCAHO standards for the provision of home infusion pharmacy services.  
LICENSURE, REGISTRATION, CERTIFICATION: Current RN license approved for practice in the State of Tennessee, CPR certification, and valid driver's license.  
EXPERIENCE: Experience in administering IV therapies, which includes the opportunity to apply principles and practices of intravenous therapy. Experience which has provided the opportunity to develop and refine managerial skills, preferred. Evidence of good communication skills, leadership ability, integrity, initiative, and creativity preferred.
