Provide skilled nursing care to patients in their homes throughout your assigned territory.
Complete comprehensive assessments to identify patient, caregiver, and environmental needs.
Develop, implement, and manage individualized plans of care designed to improve patient outcomes and promote independence.
Serve as the primary case manager for your assigned patient caseload.
Perform admissions, routine visits, re-certifications, resumptions of care, and discharge visits as appropriate.
Monitor patient progress and adjust care plans based on changing clinical needs.
Collaborate with physicians, therapists, social workers, aides, and other interdisciplinary team members to ensure coordinated care.
Educate patients and caregivers on disease management, medications, treatments, and safety strategies.
Communicate significant changes in patient condition to providers and clinical leadership promptly.
Complete timely and accurate clinical documentation in accordance with company, state, and federal requirements.
Maintain patient confidentiality and privacy in accordance with HIPAA regulations.
Support Elara Caring's mission of delivering exceptional care where patients call home.
Current, unrestricted RN license in the state of practice.
Minimum of one year of nursing experience in an acute care, skilled nursing, rehabilitation, long-term care, or other clinical setting.
Ability and willingness to travel throughout the assigned service area.
Valid driver's license, reliable transportation, and current auto insurance in accordance with state requirements.
Ability to lift, push, pull, and support up to 50 pounds, including assisting patients and moving equipment when necessary.
Associate Degree in Nursing required; Bachelor's Degree in Nursing preferred.
Previous Home Health or Hospice experience is preferred but not required.
Experience with HomeCare HomeBase (HCHB) is preferred.
