Job Description

The Clinical Intake Coordinator, LPN facilitates the Home Health & Hospice referral process by receiving referrals from multiple referral sources within and without the Saint Luke's Health System.

We are seeking a compassionate and dedicated Clinical Intake Coordinator/LPN to join our supportive environment where patients and staff are valued.

• Monday – Friday, 9 a.m.-5:30 p.m. One remote on-call shift required every 5-6 weeks. Rotating (remote) weekend and holiday requirement.
• Hybrid - Employees are required to work on-site 3–4 days per week, with the option to work remotely 1–2 days per week upon completion of orientation. Remote days are scheduled on a rotating basis to ensure adequate office coverage. Work arrangements are subject to change based on business needs.
• Determines need for additional information from referral sources
• Communicates agency acceptance or decline of referral to varied referral sources, and processes accepted referrals.
• Prioritize and triage hospice referrals.
• Ensure regulatory compliance is met during initial screening and intake process.
• Missouri and Kansas LPN License Required.
