About the role
The After-Hours Homeless Outreach Team (HOT) offers low barrier healthcare and support to people experiencing homelessness outside usual business hours, improving access to care when fewer services are available
The Peer Support Worker (PSW) is a member of HOT and provides peer led support to clients utilising a strength based, trauma informed framework, meeting people where they are at, and offering care wherever they are
The role requires resilience, flexibility and a persistent, consistent approach, with the ability to work both independently and collaboratively in dynamic outreach settings
The role also involves working in partnership with a range of services — including Specialist Homelessness Services, GPs, Homes NSW, local council and other providers — to support people to access healthcare, housing, and support services of their choice
You will also contribute lived and/or living experience and expertise to HHS research and HHS policies and procedures, strengthening the service and improving outcomes for people and communities experiencing homelessness
About you
Current unrestricted NSW Drivers License
Lived or living experience of homelessness
Demonstrated understanding of the impact of homelessness on health, and the barriers people may face when accessing healthcare
Demonstrated ability to provide strengths-based, culturally safe, and trauma-informed support, and to assist people experiencing or at risk of homelessness to access health services of their choice
Demonstrated ability to work collaboratively within an interdisciplinary team and with internal and external stakeholders
