In Med History, our goal is to obtain the best possible medication history for each patient admitted to the hospital. This step is crucial to completion of Joint Commission accreditation-mandated medication reconciliation. Medication History Technicians claim patients from a work list based on the patient’s location and the tech's scheduled shift assignment. Technicians follow a structured interview with the patient or caregiver and document the information into the electronic medical record. Discrepancies are communicated directly to the provider/pharmacist for resolution.

Job Summary

Provides technical assistance in the documentation and clarification of medication history lists under the direction of the pharmacist. Processes, enters, and interprets physician/nurse medication histories, under the direction of the pharmacist, and completes full history documentation in computerized system. Clarifies information through communication with providers, patients, and pharmacies. Performs special projects for the department. Works with multiple medication automation systems.

Essential Functions

  • Provides professional customer service as a liaison between the pharmacy department and patients/caregivers. Interacts with prescriber as needed.
  • Processes physician/nurse/pharmacist obtained patient histories and enters into the computer system and assists in maintaining patient profiles.
  • Reviews profile for allergies, duplication of drugs, and last doses and brings these to the attention of the pharmacist.
  • Checks and verifies accuracy of patient profiles.
  • Maintains clean and organized work area.
  • Maintains all requirements for CPhT certifications. Completes all Spectrum Health assigned competencies by assigned due dates.
  • Perform quality assurance data collection, collation, and presentation.

Qualifications

  • Required High School Diploma or equivalent
  • Preferred Associate's Degree health care or related field
  • 1 year of relevant experience Pharmacy Required
  • 2 years of relevant experience Retail Pharmacy Preferred
  • LIC-Pharmacist Intern - STATE_MI State of Michigan required Or
  • LIC-Pharmacy Technician - STATE_MI State of Michigan required And
  • CRT-Pharmacy Technician, Certified (CPHT) - PTCB Pharmacy Technician Certification Board 90 Days required Or
  • CRT-Exam for the Certification of Pharmacy Technicians (ExCPT) - NHA National Healthcareer Association 90 Days required
  • Ideal candidate will possess Full Pharmacy Tech License and/or National Certification, such as PTCB Cert.
  • Medication History Pharmacy Technician experience strongly preferred
  • Eligible to pick up Virtual Shifts after obtaining adequate On-Site Experience
