PT Weekend Licensed Practical Nurse (LPN) Clinical Field Staff Liaison
Schedule: Saturday - Sunday 8:00am - 5:00pm and triage from 5:00pm - 8am. Work 2 holidays per year

SUMMARY:
The LPN Clinical Field Staff Liaison (CFSL) Evening/Weekend provides support and acts as a resource to clinicians in the field. The CFSL works in the field with staff and patients and completes visits as needed. ASHN adopts the LPN scope of practice as outlined by the state for its licensed practical nurse personnel. The LPN is responsible for all practices and duties within the scope of practice as outlined by the state.
KEY RESPONSIBILITIES:
Responsible for covering workflow and supervising patient care needs during evening/weekend shifts and provides patient care, per agency needs
Triage incoming agency calls afterhours/weekends and when warranted, will write orders to add on call visits, schedule to appropriate on call staff and communicate with following physicians.
Monitors evening/weekend assignments to align with agency census and potential afterhours visits.
Serve as a resource for field staff pertaining to clinical issues and/or concerns during evening/weekend hours.
Facilitate calls to and from Field staff; uses resources to resolve issues and reports anything of concern to physician and/or manager as needed.
Work in the field with staff and patients as needed
Review Field staff STEPS performance
Maintains positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families
Assist with monthly Case Manager, Nurse and Therapy meetings
Focus on Outcome Monitors as assigned
Participates in managing cost of good working with monitoring supply costs and visits per episode
Knowledgeable of Utilization Management workflow and completes timely.
Review relevant reports: including next day high priority visits for assignment.
Reviews clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
Maintains active LPN license in state of employment
Participates in on-call, evening/weekend shifts and provides patient care, per agency needs
Participates in in-service program development
Maintains a professional appearance as a representative of the company
Understands and practices policies and procedures for agency operation
Other duties as assigned
QUALIFICATIONS:
Two years of experience as an LPN in an acute care setting. Minimum one-year home health experience.
Ability to create positive impressions and communicate with a variety of people. Maintain effective communication with patients, families, physicians and co-workers. Ability to remain calm, have patience and be accommodating. Compassionate and caring while working with patients.
Knowledgeable on nursing best practices and the scope of practice for a nurse as outlined in Chapter 4723 ORC for its LPN personnel. Ability to make appropriate nursing judgments. Ability to identify a situation and handle it with the best possible solution. Detail-oriented and observant.
Disciplined style of work ethic. Ability to prioritize and be timely. Works efficiently.
Ability to follow directions and work as a team member.
Arrives at assigned location on scheduled work day. Works according to designated hours.
Performs assigned duties with use of computer and phone.
Completes and submits all required documentation within specified company requirements.
Dexterity & vision to complete documentation on computer.
Valid driver’s license and auto insurance with your name as a listed driver
EDUCATION AND CREDENTIALS:
Licensed Practical Nurse with current license in the state of employment. Compact licensure, for states that participate, is required to be obtained within first 30 days of employment for additional support in all ASHN affiliated agencies.
