1. Patient care

  • Provide standard nursing care of dialysis patient as plan to patient in pre, post and throughout treatment under supervision of Renal Nurse Specialist.
  • Assist Physician during patient round, patient examination or procedure.
  • Deliver nursing care to patient as physician’s treatment order under supervision of Renal Nurse Specialist.
  • Document patient condition, treatment and progression completely.
  • Review, evaluate and report Laboratory result.
  • Demonstrates competency in RO system, setting-up, operating the dialysis machine.
  • Assesses patient’s condition, body weight and document in hemodialysis sheet with appropriate treatment plan.
  • Demonstrates competency in operating dialysis machine for double lumen or CVC patient.
  • Performs venipuncture AVF, AVG in order to initiate the dialysis under supervision of Renal Nurse Specialist.
  • Assessed patient’s condition and body weight post treatment to assure patient safety before discharge.
  • Educates and evaluates patient and family knowledge of kidney disease, treatment modalities and treatment plan.
  • Prepares dialysis machine and medical supply for the next day.
  • Performs other professional nursing activities as approved by Head Nurse.Demonstrates willingness to evaluate own practice.
  • Keeps abreast of current knowledge.
  • Avails self of continuing education programs.
  • Attends company’s education courses or conference once a year.

2. Quality and safety

  • Provide nursing care and service to all patients with patient safety standard.
  • Document nursing record completely as professional standard
  • Checks and prepares readiness of emergency cart completely.
  • Checks and prepares readiness of Treatment cart.
  • Checks medical supply and stock
  • Provides for patient safety in the clinical environment.
  • Record incident and customer feedback and report to head nurse.
  • Demonstrates willingness to initiate or participate in unit quality activities for safety culture.
  • Innovates and initiates in unit CQI project development.

3. Medication administration

  • Prepares medication as treatment plan.
  • Provides Medication administration for dialysis patient as medication safety standard.
  • Documents medication administration correctly and completely as medication safety standard and policy.
  • Review medication and blood value with patients every 1 month.
  • Review completeness of standard medication order in Medical Record sheet and encourage involved staff to complete record as standard.

4. Communication and service

  • Communicates effectively with patient and family and ensures that the patient’s problem and feedback receive the rapid response.
  • Communicates effectively with multidisciplinary team.
  • Provide service to patient and family with quality of care.
  • Demonstrates willingness to take a good care and provide instruction to patient with excellent service behavior.
  • Provide excellent service and coordination with multidisciplinary team.
  • Complies with accepted dress code.
  • Maintains a professional image among colleagues and to the public.

5. Education and learning

  • Demonstrates willingness to evaluate own nursing practice.
  • Keeps abreast of current nursing literature
  • Avails self of continuing education programs.
  •  Attends Thai Nephrology Nurses Society meeting or conference annually.
  • Attends company’s education courses or conference as requested
  • Exchange knowledge and skill sharing with team once a year.
