Behavioral Health RN Clinical Team Manager

Branch: Worcester, MA | Position: Full-Time | Schedule: M-F 8-5

Key Responsibilities

As a Behavioral Health Clinical Team Manager, you will:

  • Plan, direct, and oversee the delivery of home health services for behavioral health patients.
  • Coordinate patient care, including assignments, referrals, and communication with providers and families.
  • Ensure patient care plans are continually assessed, developed, implemented, and updated.
  • Maintain compliance with regulatory guidelines, ensuring timely initiation of care.
  • Process and coordinate physician orders, staff assignments, and care team communication.
  • Review and approve patient care plans and evaluate clinical appropriateness of changes.
  • Integrate input from all involved physicians into a cohesive, patient-centered plan of care.

Qualifications

  • Education: Associate’s Degree in Nursing or related field (required).
  • Experience:
    • 2+ years as an RN in Home Health or Behavioral Health.
    • 1+ year of supervisory experience.
  • Licensure: Current Registered Nurse (RN) license in [Insert State].
  • Technical Skills:
    • Proficiency in Workday and HomeCare HomeBase.
    • Proficient with MS Office Suite (Word, Excel, Outlook).
