Coverage: Three River's Area, MI

Schedule: M-F 8-5

What You’ll Do


As a Registered Nurse RN Case Manager for our Hospice service line, your day-to-day will be filled with meaningful and impactful work:

  • Coordinate and manage patient care across the continuum, ensuring the right care is delivered at the right time and place
  • Develop, implement, and adjust care plans based on patients’ changing needs, making sure they receive quality, compassionate care
  • Collaborate with physicians, families, and interdisciplinary teams to achieve the best possible patient outcomes
  • Provide emotional and physical support to patients and families throughout the hospice journey
  • Handle documentation, assessments, and deadlines while balancing multiple priorities
  • Act as a mentor and leader to other healthcare professionals on your team

What You’ll Need


We’re looking for Registered Nurses who are passionate, dedicated, and ready to make a lasting impact:

  • Graduation from an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate)
  • Current RN State License in good standing
  • At least 1 year of clinical nursing experience
  • Experience in hospice or home health preferred
  • Strong organizational skills and the ability to handle multiple priorities and deadlines
  • Reliable transportation for travel within your service area
