POSITION PURPOSE:  

Goal is to enhance and improve the clinical experience of the patient/families. Nurse Navigator will coordinate and collaborate with multidisciplinary teams to develop an up to date patient care plan and communicate this plan with the patient/family.

ESSENTIAL FUNCTIONS:

  • Greet the patient and establish a relationship by explaining the Nurse Navigator role to patient/family/caregiver.
  • Assess and identify the learning needs of the patient/family/caregiver and implement appropriate and effective teaching methods.
  • Work closely with members of healthcare team to facilitate individualized plan of care for discharge/ follow-up. 
  • Serve as the bridge between the patient and healthcare team by providing support and guidance to patient/family/caregiver.
  • Collaborate with other staff regarding discharge calls and communication to transition to long term care, home nursing or Community Health Network.

SPECIALIZED KNOW-HOW & REQUIREMENTS

  • Current Illinois Registered Nurse Licensure required.
  • Minimum of 2 years nursing experience in acute patient care strongly preferred.
  • BLS required within 30 days of hire and maintained throughout employment at CGH.
  • Ability to develop and successfully implement teaching plan based on patients’ unique and individual patient needs strongly preferred.
  • Ability to utilize computerized patient care management system (Cerner, Next Gen, MR View, and Midas) and ability to access outside sources as necessary for patient education strongly preferred.
  • Basic Computer knowledge strongly preferred.
