DEPARTMENT SUMMARY: St. Charles Center for Women’s Health department serves as the primary office for Women’s Health services. The Center for Women’s Health office is the only OB/GYN office within St. Charles and is the only office outside of Bend that provides comprehensive Women’s Health services in Deschutes, Crook, and Jefferson Counties. Our care team consists of Physicians, Certified Nurse Midwives, Nurses Practitioner’s, and High-Risk Nurse Navigators. The primary office is located in Redmond with outreach offices in Prineville, Bend, Sisters and Madras.
POSITION OVERVIEW: The St. Charles Women’s Health Certified Spanish Interpreter and Community Health Coordinator assists in teaching women and families about behaviors that promote wellness. This role serves as a trained advocate to develop and implement strategies that improve the health of individuals and communities. In addition to providing interpreter services for our Spanish speaking patients, the caregiver in this position coordinates care to help bridge the gaps and eliminate barriers to health care and social services for identified at-risk clients. They empower individuals to access community resources through referrals, education, and outreach and assist the health care team in recognizing potentially serious problems, thus preventing poor health and social outcomes for individuals and communities. This position does not directly manage any other caregivers.
ESSENTIAL FUNCTIONS AND DUTIES:
Working in collaboration with the High-Risk Nurse Navigator and provider care team, assists in the care coordination of patients who have high-risk conditions, with a primary focus on obstetrical patients.
Provides Spanish interpreter services by participating in all patient visits. Supports care team with timely communication, inquiry, follow-up, and integration of information into the care plan.
Proactively serves as the point of contact and advocate for patient, family, care team, payer, and community resources.
Contacts identified clients by telephone, mail, and/or hospital or home visit. Makes additional follow-up contacts with clients as necessary.
Arranges transportation to critical medical, behavior and resource appointments as necessary.
Cultivates effective partnerships and works collaboratively with all members of the care team including patient and family.
Empowers individuals to access appropriate prevention and wellness services to develop their own self-care.
Complete global needs assessments to identify needs and potential eligibility for any programs and/or assistance for clients as necessary.
Assists the health care team in recognizing potential serious problems, in prevention of poor health and social outcomes for individuals and communities.
Provides cultural mediation between communities and health and human services system where appropriate.
Complies with all documentation requirements. Thoroughly and appropriately documents all work completed within the medical record.
Facilitates access to health-related services, including but not limited to: assisting clients with obtaining a medical home, providing instruction on appropriate use of the medical home, assisting patients in overcoming barriers to obtaining needed medical care and social services.
Assists with identification of Primary Care Provider (PCP) / Specialist for follow-up appointment(s), as appropriate.
Provides choice lists to patient / caregiver for post-acute services.
Schedules follow up appointment(s) with PCP / Specialist / post-acute provider, as appropriate.
Establishes and maintains relationship interdepartmentally as well as within assigned department to ensure customer and caregiver satisfaction with referrals/admissions/appointments.
Maintains a thorough knowledge of community resources. Develops and maintains strong working relationships with referral sources and community agencies.
Educates clients about available community resources and works to connect clients to local social service agencies. Provides relevant educational materials and social support.
Participates in community meetings and events with other agencies and organizations regarding community needs. Attends continuing education classes and reports information and/or trains peer Certified Spanish Health Care Interpreters.

EDUCATION:
Required: High school diploma or GED.
Preferred: Bachelor’s degree in psychology, sociology, social work, or other related health care field.
LICENSURE/CERTIFICATION/REGISTRATION:
Required: Completion of a Spanish Health Care Interpreter training through the Oregon AHEC Program, Portland Community College (PCC) or a similar program that meets the training requirements for the State of Oregon.

Required: Two (2) years’ experience working directly with at risk populations.

PERSONAL PROTECTIVE EQUIPMENT:
Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.
ADDITIONAL POSITION INFORMATION:
Proficiency or ability to become proficient in word documents, excel and utilization of electronic health record.

Understanding of federal, state, and local programs and their eligibility requirements.
Ability to be pro-active in asking for help with multiple clients and organizations-churches/shelters, non-profits, grants.
