Job Description 

Service Coordination

• Provide in-depth training and onboarding education for scheduling for new Coordination Staff with assistance of On-site Trainer

• Monitor and provide coaching and performance management to Coordinators

• Assist in reviewing, supporting Procura Coordination training resources

• Formulating plan and follow through with training to address new and existing Coordinator knowledge gaps

• Proficient in coordination position to fill in on a desk for coordination where absences occur.

• Runs and reviews daily, weekly monthly reports and or provide action plan to reduce missed care because of coordination error, unfilled visits, incomplete care plan entries, overtime hours, under-utilized worker schedules and work with recruiting to review workforce requirements

• Create and maintain work schedules for Coordination Teams

• Works with Coordination teams to ensure all vacation and time off requests have coverage for patients in consultation with Supervisors

• Has full understanding of Collective Agreement Language and requirements as it pertains to Coordination where applicable

• Monitor key metrics such as missed care, canceled visits, and referral acceptance and hold coordination team accountable to meet set targets.

• Daily review tracking tools and follow up with Coordinators to ensure compliance.

Information Management

• Verifies integrity of data and makes corrections as necessary.

• Produces schedules and reports on a regular basis, and upon request of the Supervisor/Manager.

• Records all communications and consequent actions in appropriate fields/locations.

• Responds to funder reporting requirements as required.

Customer Service

• Communicates regularly with Patients/caregivers, PSS/Nursing Workers, funders and various Corporate or local departments (i.e. Accounting, People & Culture, Information Technology)

• Provides reports to appropriate professionals regarding Patient and PSS/Nursing status.

• Collaborates with accounting to ensure accurate and timely billing and payroll functions.

• Provides support and general direction to PSS/Nursing Workers, as appropriate.

• Receives concerns from Patients and PSS/Nursing Workers with regards to scheduling practices and redirects to appropriate professional.

*Performs other duties as required.

WORKING CONDITIONS

Effort (Mental & Physical):

• Working conditions are normal for an office environment

• Lifting or moving up to 10lbs may be required

• Manual dexterity required to use desktop computer and peripherals

• Communicating using multiple electronic devices (i.e. phone, email, computer and internal database)

• Will be expected to work in a team and individual setting

• Non-traditional hours, including evenings and weekends are required.

Rotational shifts are required based on branch operational requirements

Health & Safety

• Adhere to the Occupational Health & Safety Act and ParaMed Occupational Health and Safety policies within all district offices

QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Minimum qualifications for this position are one to two years of post-secondary education and one to three years of job-related supervisory experience, or equivalent combination of education and Experience. Strong proficiency in all aspects of Microsoft office, with proven skills in Word and Excel;

• Experience in Procura is an asset;

• Keen attention to detail, ability to prioritize, multitasking skills and organizational skills.

• Experience in customer service is an asset

• Experience in the health care field an asset
