As a Registered Nurse on our team, you'll provide care management/patient care coordination. Your role is essential as you are often the first individual that helps patients begin their journey Banner MD Anderson as you handle referral of patients not yet established with our providers at Banner MD Anderson. You will Collab with navigators and social workers to ensure patients have the resources they may need, ensure patients are scheduled with the appropriate provider and that prior records and imaging are available before consults.

Schedule: Remote, 40hrs. Flexible to work 5'8hr shifts or 4'10s. Hours between 6:30am-5:30pm.

Banner MD Anderson Cancer Center Located in Gilbert, Ariz. (the Phoenix Metro area) on the Banner Gateway Campus, the center provides world-class care for oncology patients - both inpatient and outpatient - and has also brought leading oncology programs to the Banner Gateway campus including designation as a Stem Cell Transplant Center of Excellence and comprehensive Head & Neck cancer care. Our capabilities include five linear accelerator vaults, a brachytherapy vault, an advanced diagnostic imaging suite with PET/CT scan, more than 70 infusion bays, a cryopreservation lab and much more. Our inpatient medical oncology unit also incorporates a program that utilizes the electronic surveillance partnership in caring for the patient, where remote nurses have the ability to interact with patients via two-way audio-video to assist the bedside nurse with patient care.

POSITION SUMMARY
This position provides comprehensive care coordination for the initial patient access process to ensure appropriate care alignment and a smooth and supportive patient experience. This position collaborates with a multi-disciplinary team to ensure timely diagnosis and treatment to optimize patient outcomes.

CORE FUNCTIONS
1. Coordinates the initial patient access process, making appropriate referrals and consultations in coordination with physicians and providers. Evaluates the medical necessity, urgency and appropriateness of care, optimizing patient outcomes. Collaborates with the patient and referring physician to solicit information needed to determine appropriate physician alignment for consultation.

2. Assesses the medical records to determine if adequate and appropriate records have been obtained. Determines if additional records and pathology reviews are required for initial consultation and collaborates with appropriate resources to secure needed records and documentation.

3. Assists the patient/family through the initial patient access process, providing support and updates. Stays aware of the patient’s progression throughout the intake process. Responds to patient requests for information regarding the initial consultation and general questions about the facility, physician and/or available services and resources.

4. Provides support and education to the community physician practices and prospective patients about the referral process. Serves as a resource to other staff members and provides education as needed.

5. Supports process improvement activities for populations of patients to achieve the optimal clinical, financial, operational, and satisfaction outcomes. Participates in staff development to maintain current standards of practice and ensure the highest quality of care.

6. This position has responsibility for coordinating the initial patient access process. This position works independently, with freedom to determine how to best accomplish functions within established procedures. This position confers with supervisor on any unusual situations. This position is facility-based with no budgetary responsibility. Internal customers include all levels of staff, including physicians. External customers include patients, families, employees of other healthcare institutions, physician offices, referring physicians, community providers and agencies, payers, provider networks, and regulatory agencies.

MINIMUM QUALIFICATIONS
Must possess a current, valid RN license in state of practice, temporary RN license in state of practice, or compact RN licensure for current state of practice. Additional certification or continuing education may be required based on area of practice.

Requires knowledge as typically obtained through a minimum of three years of current experience in specialty area. Requires excellent organizational skills, effective communication and writing skills, good time management skills and knowledge of word processing and database software applications. Also requires a good understanding of process improvement.

PREFERRED QUALIFICATIONS
Bachelor’s degree in nursing or related field is preferred.

Certification for BLS is preferred.


Additional related education and/or experience preferred.
