Clinical Triage Specialist – Heart Failure Program – Midtown

Join our growing team in this exciting new role as a Clinical Triage Specialist in the new Centralized Heart Failure Management Program. You will play a pivotal role in patient care, partnering closely with our physician assistants, physician leaders, and care team to remotely monitor heart failure patients using medical technology. Your responsibilities will include reviewing alerts, responding to calls and emails from patients in the program, documenting in the electronic medical record, creating medication refill orders for sign-off, supporting heart failure medication titration as indicated by the provider, and communicating with clinical staff to ensure appropriate and ongoing care coordination.  This position is for someone highly motivated, innovative, and interested in being at the forefront of expanding a newly created program’s reach. 

This is a full time, days position located at our corporate office in Midtown Manhattan.
 

Preferred Criteria

  • Master’s degree
  • RN Licensure in NJ, FL, CT and/or Compact

Required Criteria

  • Bachelor's degree or equivalent work experience
  • Current NYS RN License
  • Effective in a fast-paced environment and demonstrates strong problem-solving skills
  • Excellent communication skills
  • Ability to handle sensitive and confidential information according to internal policies
  • Ability to set and meet deadlines, prioritize work, and be able to work independently
  • Ability to work with technology as necessary to complete the job effectively

$121,000-$156,000/Annual
