Job Summary

Under the direct supervision of the Nursing Manager, and in collaboration with the interdisciplinary team, the Registered Nurse uses clinical judgment skills to access and implement the treatment plan for patients. The RN assesses monitors, detects, plans and takes appropriate action to prevent physiological complications associated with specific health problems and/or medical treatment.


Essential Functions

  • Communicates effectively with staff, residents and families. Assesses, develops, and maintains an individualized patient plan of care with the patient.
  • As needed, specified by SH entity: Performs Medication Administration Pass according to regulatory guidelines. Informs MDS coordinator of significant resident/patient changes needed for completion of the MDS.
  • Communicates with other members of the health care team in order to ensure continuity of care and coordination of services. Acts as a patient/family advocate.
  • Evaluates patient’s and significant other’s educational needs and develops and implements appropriate teaching strategies or makes appropriate referrals.
  • Observes and reports the patient’s condition, needs and outcomes of intervention to the physician and other team members. Identify problems and emergency situations, and initiate immediate “life saving” measures in the absence of a physician.
  • As needed, specified by SH entity: Teaches and supervises the care provided by CNA, LPN or other nursing service personnel according to regulatory and agency guidelines.
  • Implements the plan of care by providing nursing services, including teaching, treatments, diagnostics, and preventive procedures, regularly evaluating the patient’s progress toward established goals recommending revisions to the plan as necessary, and planning for termination of service or discharge. Assumes a leadership role and delegates appropriately to support staff to facilitate safe, quality patient care in a cost-effective manner.
  • Participates in ongoing process improvement activities through the application of clinical paths, QA processes and other related activities. Ensures that processes and services are continuously monitored for quality, cost effectiveness, and efficiency.

Qualifications
  • Required Associate's Degree Nursing
  • Preferred Bachelor's Degree Nursing
  • 2 years of relevant experience OB-postpartum and breastfeeding support, specific to the Going Home RN for Healthier Communities Required
  • 1 year of relevant experience of acute care, emergency, or rehab facility experience for all of the Home Care business lines Preferred
  • LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire required
  • LIC-Driver's License - STATE_MI State of Michigan Upon Hire required
  • CRT-Basic Life Support (BLS) - AHA American Heart Association VNA and Home Care Business lines 90 Days required Or
  • CRT-Basic Life Support (BLS) - ARC American Red Cross VNA and Home Care Business lines 90 Days required
  • CRT-Board Cert Lactation Consultant (BC-IBCLC) - IBCLCE International Board of Lactation Consultant Examiners Going Home RN for Healthier Communities Upon Hire preferred
