As a Licensed Practical Nurse (LPN), Hospice the work you do every day makes a difference in the lives of our patients. Our nurses give our patients the greatest gift – the ability to spend enhanced quality time with their loved ones in their preferred environment.
As an LPN you will have the opportunity to provide one on one patient care and utilize your clinical skills in providing end of life care as directed by an attending physician. You'll be part of an interdisciplinary team that focuses on providing compassionate quality care to our patients.

HOW YOU'LL WORK:
You'll provide comprehensive skilled care as ordered by the attending physician and by following the order sets created by the RNCM. You will work autonomously, so you need to be efficient in managing your time. You'll be responsible for all practices and duties within the scope of practice outlined by the state.
MAJOR AREAS OF RESPONSIBILITY:
Plan of Care: Perform care by following order sets and goals made by RNCM and notifying the RNCM of any changes in patient condition.
Minimize Patient Risk: Observe and monitor patient conditions and reconcile medications prescribed by the physician.
Family Educator/Advocate: Counsel patient and family on the disease/progression and incorporate patient and family in development of plan of care. Listen to patient and family members.
Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
Operations: Attend in-service training and mandatory agency meetings.
HARD & SOFT SKILLS:
Compassionate communicator with a positive attitude.
Patience is a virtue when working with patients, families, physicians, and coworkers.
Attention to detail is critical, as is being observant and following directions.
REQUIREMENTS:
Licensed Practical Nurse (LPN) with current license in the state of employment
Minimum one-year experience as an LPN in an acute care setting is highly recommended.
Hospice experience is desired.
Valid driver's license and auto insurance in your name as a driver
Capable of all physical demands
