What You’ll Do

As part of our Transitions of Care Team, this role assists patients in getting post-hospital care in the form of telephonic and in-person patient support.

  • Work as part of an interdisciplinary, collaborative team to ensure we are meeting patient needs
  • Assist patients in enrolling in Imagine Pediatric programs
  • Round at facilities in your geographic area to meet with Imagine Pediatric patients and caregivers
  • Coordinate with facility discharge planners to ensure that there are no barriers to patients obtaining after-hospital resources and care
  • Attend daily huddles with your team to communicate patient needs and progress
  • Liaison with hospital and other health systems to make staff aware of Imagine Pediatric program and support
  • Supporting patients with any in person/community needs related to care coordination/discharge support. These could include, but are not limited to, pharmacy fills, PCP coordination, SDOH needs, and home visits.

  • Ability to adapt to changes

  • Excellent written and oral communication skills
  • Strong critical thinking skills
  • Ability to manage multiple tech platforms
  • Experience working in a patient-facing hospital or facility role
  • Experience working as part of an interdisciplinary team
  • Ability to organize and prioritize daily schedule
  • Valid Texas driver’s license
  • Bachelor’s degree
  • Bilingual (Spanish) highly preferred
  • Texas or National CHN certification (obtained within 6 months of hire date)
  • CPR certified
  • 3+ years of experience in a healthcare setting
