As a Department Manager for the Post-Acute Care Program, you will oversee operations, staff, and workflows while ensuring the delivery of high-quality, value-based care. In this role, you will plan, develop, and maintain systems and procedures and help day-to-day operations to support efficiency. You will also serve as a liaison between departments, outside facilities and community partners while cultivating a culture of innovation and excellence within the Post-Acute Care team. A caregiver in this role works remotely from 8:00 a.m. – 4:30 p.m. Candidates must be local to the Northeast Ohio area, as some on-site meetings will be conducted in Independence and/or Beachwood.
A caregiver who excels in this role will:
Manage department personnel and operations which includes staffing, training, workflow analysis and performance management.
Manage staffing to ensure appropriate allocation and compliance with goals and objectives.
Monitor quality and financial performance.
Ensure compliance with government health regulations, JCAHO guidelines, quality requirements and customer service standards.
Recommend enhancement and cost saving strategies.
Resolve interdepartmental issues to ensure consistency in quality, procedure and policy application.
Develop, monitor and implement programs to enhance customer service, develop skills and improve scope of service.
Analyze programs and make improvements.
Participate in managers’ meetings and relevant committees as assigned.
Lead meetings to review program progress and collaborate with skilled nursing facilities (SNFs) regarding their performance.
Manage staff responsible for on-site SNF visits and patient navigators who call and follow up with patients.
Minimum qualifications for the ideal future caregiver include:
Bachelor’s degree in Business Administration, Health Care Administration or related field and six years supervisory or management experience in a related setting OR Master’s degree and five years of experience
Proficient in EPIC, with ability to troubleshoot and navigate the system
Experience using Tableau dashboards for reporting
Experience leading SNF teams, including administrators, assistant administrators and corporate staff, with a strong background in SNF operations
Experience working in any skilled nursing facility (SNF) setting
Preferred qualifications for the ideal future caregiver include:
Post-acute experience, particularly in skilled nursing facilities (SNF)
Strong understanding of SNF operations and local post-acute market details
Prior managerial experience with strong analytical skills
Healthcare experience

Physical Requirements:
Manual dexterity sufficient to operate office equipment.
May require extended periods of sitting, standing and walking.
Must be able to walk to attend meetings; must have normal or corrected vision to normal range.
Ability to clearly communicate by phone and in person.
May occasionally lift up to 25 pounds.
