Your Job:

The Faith Community Health and Wellness Promoter will provide intermediary support for the MHS Faith Community Nursing Team to improve and facilitate engagement with MHS Faith Community Covenant Partners using evidence-based education and the Westberg Institute for Faith Community Nursing Principles along with coordinating our efforts with the MHS Wellness program. Functions as an active team member of the Faith Community Nursing staff.

Your Job Requirements:

• Degree in Nursing, Public Health, Healthcare Administration or healthcare related field required
• 4 years of relevant process improvement and project coordination experience may be considered in lieu of degree in addition to the experience below.
• Excellent communication and interpersonal skills. Good oral, written, and presentation skills. Bilingual (English/Spanish) a plus.
• Demonstrates ability to identify and effectively address dynamics that threaten success of a project or present barriers to implementation. 
• Ability to self-motivate, establish priorities, and invest in a change process to improve efficiencies.

 
Your Job Responsibilities

• Establish and manage relationships with Faith Community Nurses, faith community leadership, service organizations and community groups.
• Develop and maintain methods to monitor program activities and reporting to ensure accuracy in outcome measures.
• Serve as programmatic liaison to internal and external stakeholders.
• Participate in continuous quality improvement activities.
• Identify strategic partnership opportunities and promote relationship-building.
• Disseminate programmatic and marketing materials.
• Develop presentations and other materials for use during program-related events. 
• Participate in regular team meetings and peer review activities. Participate in departmental and organizational committees as applicable.
• Develop and maintain strong partnerships with hospital and community stakeholders to support population health programs within the organization and address improvements in health and well-being of the community.
• Create and maintain up to date work flow and reporting documentation. Act as a liaison between clinical users and IT management to maximize and support appropriate use of application(s).
