Position Summary

Required to live in Long Island, New York, or within commuting distance of Long Island.

50% of time will be spent in provider offices in Long Island, NY to providers.
 

This will be a hybrid position which will include telephonic outreach to beneficiaries as well as travel to skilled nursing facilities to collaborate with discharge planners and assess beneficiary for immediate needs prior to discharge.

Schedule Standard business hours. No nights, no weekends and no holidays.

The Health Coach Associate assists in the collaborative process of assessment, planning, implementation and evaluation, to engage and educate members in obtaining or maintaining optimal health status. 

Assessment of Members through the use of clinical tools and information/data review, conducts an evaluation of member's needs and benefit plan eligibility for available internal and external programs/services and facilitates integration.

Contributes to the overall assessment of a member, including member’s readiness to change, by utilizing focused assessment tools to periodically review member’s health status to determine level of intervention.

Uses judgment to determine when a case is triaged to the RN for more comprehensive oversight.

Enhancement of Medical Appropriateness & Quality of Care.

Application of criteria and guidelines, policies, procedures, regulatory standards to ensure appropriate utilization of services and/or administration and integration with available internal/external programs.

Consults with supervisors, Medical Directors and/or others to assist members in meeting goals. Identifies and escalates quality of care issues under the guidance of a Registered Nurse.

Utilizes influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum level of health.

Provides coaching, information and support to empower the member to make ongoing independent healthy lifestyle choices. Monitoring, Evaluation and Documentation of Care.

In collaboration with the registered nurse and the member (and, if applicable, the attending physician), assists in establishing, implementing and monitoring plans of care to meet the member’s goals.

Utilizes National Care Management and/or internal Policy and Procedure in compliance with regulatory and accreditation guidelines.



Required Qualifications

  • Requirement to reside in Long Island, New York, or within commuting distance of Long Island, NY.
  • 3 years of clinical experience required.
  • A LPN/LVN that must hold an unrestricted license in their state of residence, New York, with multi-state/compact privileges and have the ability to be licensed in all non-compact states.
  • Depending on requirements of SNFs, certain vaccines (i.e., COVID, Flu) may be a requirement of the role, as well as additional health screenings (i.e., drug screen, vaccine titers).



Preferred Qualifications

  • Previous coaching experience preferred.
  • Previous work with Medicare beneficiaries preferred.
  • Bilingual - English/Spanish preferred.

Pay Range

The typical pay range for this role is:

$23.69 - $50.98

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
