Job Description​

The Opportunity:

We are hiring an experienced APP for our Plaza Primary Care Clinic in Kansas City, MO.

The Work:

• This position is accountable for advanced nursing practice in the care of assigned patient populations
• Responsible for collecting and analyzing client health data
• Identifies appropriate expected outcomes
• Develops and implements patient plan of care
• Provides comprehensive clinical coordination of care and case management
• Provides consultation to influence plan of care for clients using prescriptive authority procedures and treatment in accordance with state and federal laws and regulations
• Evaluates clients progress in attaining expected outcomes
• Conducts nursing research and provides education to patient family and nursing staff
• Position will primarily see adult patient population

Candidates with 2-3 years of advanced practice preferred.

Master's Degree, Dual licensure APRN Missouri and Kansas, BLS through American Heart Association or American Red Cross required.

Advanced Practice Provider Cert(MO/KS) - Various, Basic Life Support - American Heart Association or Red Cross
