About the Role
The Grade 3A District Nurse will provide high quality nursing services including assessment of broader care needs to a diverse community. The District Nurse is responsible for taking a wellbeing and reablement approach in improving the health status and independence of the client and supporting the carer in their caring role
Provide high quality nursing services delivered within professional boundaries and responsibilities with regard to the assessment, care planning, treatment, review and education of clients and carers.
Promote and maintain the active involvement of clients and their carers in care planning
Liaise and work co-operatively with local service providers and encourage co-ordinated case management practices, particularly to clients with chronic and complex needs, to ensure the delivery of services is seamless and appropriate to individual needs
Classification for this position will be YU1 ($99,439.60 per annum pro rata)
Your Contribution
Relevant clinical experience within the community setting and/or across a broad range of acute medical/surgical services
Demonstrated highly effective interpersonal skills including problem solving, conflict resolution, negotiation and teaching.
Experience working with clients with multiple comorbidities and complex care needs that impact safety, function and independence.
Excellent organisational, coordination and time management skills.
Current nursing registration with AHPRA
Current Driver’s Licence
Current National criminal history check, or willing to obtain and Current NDIS Worker Screening Check

About St Vincent’s At Home District Nursing Team
St Vincent’s at Home (SVAH) is a multi-disciplinary team providing 7 day/week Hospital in the Home (HITH), Cancer Care at Home (CCAH) and District Nursing services. SVAH also provides HITH care to The Cottage, a six bed short stay facility providing care to people experiencing homelessness.
District Nursing services provides district nursing care to clients under the Commonwealth Home Support Program (CHSP), Victorian Home and Community Care-Program for Younger People (HACC-PYP), Transition Care Program and is a service provider to Department of Veteran Affairs (DVA) eligible clients. The District nursing service is also responsible for our Home Based Cancer Care pathways as part of our Better @ Home funding, a key pillar of our health service Care Beyond the Walls strategy.
