Position Title: Chronic Disease Clinical Nurse Specialist Grade 2

Employment Type: Permanent Full Time

Hours per week: 38 hours per week

Salary: $109,945.46 - $113,556.22 Base Salary per annum

Enterprise Agreement: Named NSW (Non-Declared) Affiliated Health Organisations’ Nurses Agreement 2017

The position works within the Chronic and Complex Care Service, which is a large multidisciplinary team (nursing and allied health professionals) managing patients with chronic and complex clinical, psychological and social needs. This role works in conjunction with the multidisciplinary team to meet the goals of the patient (and carer) receiving community-based chronic disease services in both a clinic and home-visiting environment.

This position will be working closely with Chronic and Complex Care specialist nursing teams including Chronic Care Coordinators, Heart Failure, COPD and Chronic Wound services and assists individuals with chronic diseases such as heart and lung disease to self-manage their condition and maintain health and fitness at home to avoid hospital intervention.

About the role:

You will be working within the Chronic Disease Team providing care coordination, health coaching and education to clients who require support for managing their chronic and complex health conditions. This will be delivered in multiple clinical settings including within their own homes, using hospital pool cars.

Selection Criteria:

  • Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent’s Health Australia.

  • Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency

  • Relevant post-registration qualifications and at least three years’ experience working in the clinical area of their specified post-graduate qualification

  • Demonstrated ability to work within the interdisciplinary team at a senior level

  • Demonstrated commitment to ongoing professional development and demonstrated effective communication and interpersonal skills

  • Demonstrated effective computer literacy with email and an electronic patient record system

  • Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis in their daily practice as a registered nurse

  • Demonstrated effective time management skills and ability to prioritise competing workloads within a complex environment
